Dental Care Levels for CSHCN
On this Page
In collaboration with local providers, the Medical Home Portal developed this list of levels of severity of children and youth
with special health care needs (CYSHCN). All dentists in Utah were provided with a survey and asked to identify the types
of CYSHCN that they were comfortable treating by choosing one of the listed levels. The dental care providers that are listed
in our database responded to the survey and gave permission to be listed. Please contact the individual dental care provider
for more information about provided services and accepted insurance plans.
