Spring 2017 UCCCN Learning Session
We had a terrific Spring Learning Session on March 15th. Our topic was “Collaborating on Care Coordination.” 63 people participated,
10 of whom were from payer (or payer-like) organizations: Healthy U, Molina, Health Choice Utah, SelectHealth, and Pediatric
Specialty Services (PSS). Others were from Utah pediatric practices. There were some great questions and answers around
collaboration between care managers at the insurance organization and care coordinators in practices. Some things were clarified
and ideas for better communication were shared.

- Frequently used custom Services Providers lists – they are easy to copy and customize for your own use. Printing of lists has been improved and of course, the list link is permanent, so please share with patients, Medical Home team members, etc.
- Information in the Utah Services Directory about primary care providers for transitioning young adults with special health care needs.

What would you do without the Medical Home Portal? Our main funding ends this summer. We are exploring a variety of approaches
to sustaining the Portal, including a proposal through Utah Medicaid for federal funding to develop an “app” that could plug
in to electronic health records. We welcome your thoughts and ideas for ways to support the Medical Home Portal and its team.
