Transition to Adult Healthcare

Teen to adult transitions is one of nine system initiatives selected to receive financial support through the Intermountain 2020 Primary Promise campaign. The goal is to build a community standard for every teen living with a chronic condition and build systems of care that provide transition consistently. Because of this campaign, there is much work being done throughout Intermountain Healthcare, University of Utah Health, the Utah Department of Health & Human Services, the Utah Parent Center, and the pediatric and adult community. 

Many from these organizations, along with young adults who have transitioned and parents of CYSHCN in the community, have participated in the following efforts.
